Duration 3 months beginning April 1, 2014 to June 30, 2014. Registration fee required: $30.00. Minimum Sponsorship required: $100.00
The registration fee includes an introductory session plus 30 minutes follow up 2 weeks after with a dietician end of April, 2014 to be scheduled by appointment only. (exact date to be announced).
All proceeds go to the Island Lake Diabetes Camp.
Prizes include:
Most Percentage Weight Loss Prize – $500 Visa card plus a ten spa ($ 450.00 Package) at the Fort Garry Hotel.
Most Amount Sponsorship Funds Raised Prize – $ 500.00 Visa Card Plus IPAD.
Winners for both prizes to be announced July 9, 2014.
All funds to be submitted to our office on or before July 7th, 2014.
Weight in at the beginning of April 1, 2014 or after.
Weight in at the end June 30, 2014.
